What we collect. When you use a form on this site, we collect what you enter: your name, email, organization, role, how you heard about us, and any message. If you email us a report, we receive that report.

Why. We use this only to reply to your request and follow up about it.

Where it is stored. Form entries go to Dr. Claud’s email and are saved in the Squarespace Contacts panel. Reports you email us stay in BenefitsDoc’s business systems.

Patient information. Please do not send patient-level information. Send summary reports only, and only reports you are allowed to share.

Follow-up. Dr. Claud replies personally. We do not send automatic emails.

Website visits. Squarespace, which hosts this site, may collect basic visit information, such as which pages are viewed.
